Supporting our busy Transplant Center, this position involves working with patients and their families providing care planning, assessments, counseling and resource provision. By working together our surgeons, doctors, transplant nurses, pharmacists,dieticians, social workers,psychiatristsand othermembers of the teammanage every aspect of ourpatient’skidney transplants, from planning through post-surgical care. We comply with required standards for medical/ legal responsibilities and serve as a resource to other allied health and administrative staff within the Outpatient Social Work Department and the Department of Care Management.

Position description

Collaborates with multidisciplinary teams, provides counseling and/or psychotherapy, coordinates patient discharge planning, complies with required standards for medical/legal responsibilities, and serves as a resource to other allied and administrative staff within Outpatient Social Work and the Department of Care Management.


Requires a Master of Social Work (MSW) or Master of Science in Social Administration (MSSA) degree from an accredited social work program.

Additional qualifications

Candidates must have an advanced knowledge of mental illness, personality development, knowledge of the types of treatment, and skills in conducting individual, group, and family therapy. Must have knowledge of adjustment to medical diagnosis' and chronic disease. Strong communication skills are needed to interact with patients, patient's family members, consultants, residents, hospital personnel, community agencies, and other resources. Must also have psychosocial diagnostic skills and the ability to communicate effectively in both verbal and in written form.

License or certification

Licensure (LCSW) to practice independently at the clinical level as required by applicable state requirements.

Compensation Detail

Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum salary every 2 weeks is approx $2,257.60, based on a full-time position.
